					I have tried everything (all of the “conventional” treatments for my pain/allergies/symptoms) and have limited or no success.

					I want a health care provider who is also a PARTNER. It is important to me to be active in my health care and I want to play an active role.

					I prefer to avoid ingesting unnecessary chemicals if possible. If there is a non-pharmaceutical way of treatment, I'd prefer to pursue that route first.

					I want to be able to TALK to my health care provider about not only what is being done but also why it’s being done.

					I'm willing to make an investment of time as part of my treatment. I recognize that Traditional Chinese Medicine doesn't offer a "quick fix." This includes taking time for myself to relax, rejuvenate and restore.

					I am committed to making the necessary lifestyle changes which will enable me to not only get well but will then serve to help me stay well.

					I have multiple health issues that require seeing several different specialists and I suspect that instead of having multiple problems there could be just one or two underlying problems that could be solved through a holistic approach.

					I am open to challenging traditional "American" views on a healthy diet and nutrition and learning more about Traditional Chinese Medicine's view of good nutrition.

					My doctor(s) keeps running tests, yet they all say nothing is wrong. Despite what the tests say, I still just don’t feel well.

					I prefer a "hands on" approach and would appreciate my health care provider doing a physical exam as part of the diagnosis.
